As advances in clinical research, medical technology and life sciences create newer, better, safer, and potentially faster care protocols, the care settings where the care is delivered are changing, often moving from inpatient to outpatient or ambulatory settings, post-acute care, nursing homes, ultimately even to patients’ homes. Care is also transitioning from the traditional brick and mortar in person models to virtual medicine meeting the care receiver where they are. As the care becomes increasingly distributed, it becomes vulnerable to the variety of breakdowns in the links along the care pathway. During this discussion the experts from some of the leading healthcare organizations will share insights about how to bridge the gap between the current fragmented state of care delivery and the desired future of truly coordinated care.
